The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

POSITION OVERVIEW

 

The NYC Department of Health is seeking an Asthma Community Health Worker to provide point-of-care caregiver and family education for pediatric asthma self-management and support. The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers.

 

PROGRAM OVERVIEW

 

The Bureau of Bronx Neighborhood Health which sits within CHECW aims to enhance health, wellness and equity in the Bronx to elevate quality of life and life expectancy by:

 

  1. Creating targeted programs to address community health needs and concerns.​
  2. Supporting and empowering community members and organizations​.
  3. Generating and sharing public health research and data to champion the borough's health.​
  4. Advocating for policy reforms that will positively impact the health of the Bronx​.

 

This position is grant-funded.

 

RESPONSIBILITIES

 

  • Engage children, caregivers, their families, and school staff within a School Based Health Center in the South Bronx.
  • Conduct outreach to parents and caregivers via tabling, class presentations, and parent-teacher conferences about school-based health center and health department resources to help control asthma and mitigate environmental triggers.
  • Present to school’s principal and staff about asthma support resources offered by school-based health center and Dept of Health via presentations and one-on-one meetings.
  • Serve as a liaison between the school-based health center and the school attendance committee to identify children with active asthma.
  • Serve as a liaison between the school-based health center and primary care providers to optimize coordination of care for children whose primary care is not delivered within the SBHC’s health system.
  • Serve as a liaison between the school-based health center and asthma-focused health department staff.
  • Facilitate delivery of asthma self-management education curriculum to children and caregivers targeted to families of children with poorly controlled asthma. Assess baseline knowledge and fill in knowledge gaps using educational materials to describe physiology, triggers and medications for asthma. Provide education on Medication Administration Forms.
  • Review asthma action plans.
  • Assess for health-related social needs and refer as needed to programs such as,
    • Integrated Pest Management (IPM)
    • Refer to internal or external case management services within the SBHC’s health system or health department-funded organizations when appropriate.
  • Review medication prescriptions to ensure appropriate asthma medication.
  • Document each pediatric encounter in detail in the electronic medical record.
  • Collect and summarize data to monitor patient touches, changes in outcomes.
  • Take part in public health detailing initiatives in the South Bronx when needed.
  • Performs other duties as assigned.

 

QUALIFICATIONS

 

  • High school diploma or GED
  • Associate degree or bachelor’s degree in community health or related field preferred.
  • Bilingual (English/Spanish or English/French) is required.
  • Strong relationship builder with excellent interpersonal and communication skills. 
  • Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred. 
  • Experience working with marginalized populations preferred.
  • Healthcare related experience is a plus.

 

SALARY AND BENEFITS
