The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

PROGRAM OVERVIEW 

The Bureau of Harlem Neighborhood Health (Harlem BNH) supports the agency’s mission to protect and promote the health of all New Yorkers.  Harlem BNH catchment area includes the entire community of Harlem, including East, Central and West Harlem as well as Washington Heights and Inwood.  Harlem BNH focuses on the structural and root causes of health disparity outcomes and develop programs to address these outcomes with input from partners and residents.  Addressing the social determinants of health (SDOH), is a key focus point and is enshrined in programs such as the Harlem Health Advocacy Partnership and the Asthma Counselor Program.  Other Bureau offerings include a variety of programs and activities focused on the health and wellness of Harlem residents. The Harlem Bureau also houses the East and Central Harlem Neighborhood Health Action Center, which is a key part of the NYC's effort to promote health equity and reduce health disparities in the neighborhood.

 

As part of the NYC DOHMH Public Health Corps project, the Harlem BNH team consists of community health workers supporting all of its programs and initiatives. 

 

POSITION OVERVIEW

The Built Environment/Community Health Worker will be a key member of fast-paced teams that develops, manages projects and executes the built environment and schools work. This position focuses on the neighborhood Complete Streets/Built Environment and Coalitions work that will be housed within the East Harlem Neighborhood Health Action Center. This position is within the Center for Health Equity & Community Wellness (CHECW) at the New York City Department of Health and Mental Hygiene (DOHMH). The Center for Health Equity & Community Wellness uses a racial and social justice approach to eliminate health inequities for those who are most marginalized in New York City and to reduce overall premature mortality from the leading causes of preventable death with the vision that every New Yorker will live in a thriving neighborhood with equitable access to resources that help support healthy individuals and communities. This division unifies and strengthens the DOHMH’s lines of work directed at eliminating racial inequities for preventable health conditions, which are rooted in historical and contemporary injustices and discrimination, including racism. CHECW’s aim is to eliminate racial inequities resulting in premature mortality, with a focus on chronic disease, by addressing the social and environmental factors that impact health. The goal of this renewed approach is to increase placed-based investments in priority neighborhoods with community programming and services based on epidemiology; influence and leverage the health system to promote whole-person care; and intensify the agency’s approach to tackle big salt and sugar, big tobacco, the built environment and other determinants of health. This position will report to the Program Manager of the Health Equity Capacity Building team. This position will be located at East Harlem Center for Asthma Excellence (hybrid) and will most likely require travel to and onsite work at community and/or event sites.   

 

RESPONSIBILITIES

Built Environment/Complete Streets

  • Support the Built Environment team in the execution and development of Complete Streets/Built Environment policy and programming initiatives, including supporting 2 neighborhood coalitions.
  • Support Research and Evaluation in all evaluation efforts including data collection, surveying, assessments, literature reviews, etc.
  • Maintain and nurture relationships between coalitions, community organizations and city agencies by attending and participating in meetings.
  • Manage communication platforms, including email and social media accounts for coalitions.
  • Provide regular communications with partnering sites and assist in communications related to the program.
  • Perform other duties as assigned, such as drafting briefs, preparing presentations, conducting research, developing website content and reviewing and summarizing scientific articles.
  • Participate in and lead group bike rides and/or walks and other community events; this may include weekends.

General

  • Support community members to advocate for their needs. 
  • Hold group and individual meetings with community members in support of specific program responsibilities. 
  • Deliver health information in a way that is easy to understand. 
  • Determine eligibility for public programs and services, as needed. 
  • Collect data to inform future programs. 
  • Facilitate communication between community members and Harlem BNH and DOHMH.
  • Respond to emerging community priorities as part of CHW workforce, as needed. 

 

QUALIFICATIONS

  •  Undergraduate degree from an accredited university or equivalent experience.  Preference for candidates pursuing an advanced degree in Public Health, Urban Planning, or a related field.
  • Community engagement experience (e.g. outreach, education, etc.).
  • Experience embedding a racial equity and social justice lens in data work.
  • Very strong organizational skills and ability to handle multiple priorities.
  • Excellent written and oral communication skills.
  • Experience with community outreach and/or engagement.
  • Strong communication skills (written and verbal) required; Ability to translate and present DOHMH data, policies, and recommendations clearly and effectively to external and/community stakeholders plus.
  • Research and evaluation experience preferred, including quantitative and qualitative data collection.
  • Knowledge of public health programming and strong computer skills in Microsoft Word, PowerPoint, and Excel.
  • Knowledge of or resident of East and/or Central Harlem preferred.
  • Experience with social media.
  • Ability to work independently, be flexible, and handle changing workload with multiple projects and changing priorities.
  • Candidate should be detail-oriented, organized, and creative.
  • Self-motivated and self-disciplined, with a strong sense of teamwork and the ability to function effectively both independently and as part of diverse team of public health professionals.
  • Experience with Salesforce a plus.

 

SALARY AND BENEFITS

  • Salary range is $55,000 - $60,000
