The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

POSITION OVERVIEW

In collaboration with FPHNYC, BeHS is seeking a Practice Facilitator (PF) to join the Community Healthcare Support Team (CHST). The Practice Facilitator will be expected to work on quality improvement (QI) initiatives focused on improving chronic illness in the primary care setting, with special attention to blood pressure control, cholesterol management, and addressing diabetes prevention and management through the delivery of evidence-based lifestyle programming. In addition, the Practice Facilitator is responsible for delivering technical assistance to NYC REACH partners, at independent practices, as well as assisting in the development of workflows that will lead to desirable health outcomes in the patients being serviced by these clinical sites.

 

PROGRAM OVERVIEW

The Bureau of Equitable Health Systems (BeHS), a bureau in the NYC Department of Health and Mental Hygiene (DOHMH) is devoted to improving health by promoting and supporting primary care. The BeHS works with over 1,300 health care organizations on increasing clinical prevention opportunities, reducing health disparities, managing/preventing chronic disease, and creating community linkages to support health promotion.

Many of the activities and services to primary care are funded by grants and delivered through the New York City Regional Electronic Adoption Center for Health (NYC REACH) devoted to promoting and supporting the adoption of the chronic care model; using payment reform to advance population health by advocating and promoting reforms and programs that will facilitate the delivery of quality primary care; and monitoring the New York City healthcare system by leveraging data to monitor quality of care and population health to inform public health decisions. RESPONSIBILITIES

The Practice Facilitator will collaborate with CHST staff to support QI programs that aim to improve health outcomes and patient self-management efforts. The Practice Facilitator will serve as a primary point of contact for primary care providers and their staff. Primary responsibilities include, but are not limited to:

 

  • Managing a caseload of 30-40 practices in the engagement of quality improvement initiatives using practice facilitation strategies.
  • Conducting on-site or remote visits to provide technical assistance to practices improving patient health outcomes, particularly for hypertension, pre-diabetes, diabetes, smoking, and other cardiovascular-related priority health areas.
  • Providing support for workflow redesign and EHR documentation to improve office efficiency for care management.
  • Providing coaching on preventive-health features of EHRs and how to use them as part of QI activities.
  • Serving as a liaison between primary care practices and self-management programs for chronic illness.
  • Acting as a content expert for evidence-based intervention (EBI) referral workflow development in small practices.
  • Participating in forums discussing performance feedback and sharing best practices for QI efforts and practice transformation programs.
  • Serving as a liaison between practice staff and various BeHS teams to provide feedback on QI efforts.
  • Delivering presentations and trainings to internal and external audiences.

 

QUALIFICATIONS

Minimum Qualifications

  • Two plus years of relevant work experience including involvement in quality improvement programs, Patient Centered Medical Home (PCMH), and Quality Payment Program (QPP) initiatives at health care practices
  • Ability to travel within the 5 boroughs of NYC
  • Advanced skills in data management as well as the ability to analyze and present data to groups and individuals

 

Preferred Qualifications

  • Bachelor’s degree; Master’s degree a plus
  • Two plus (2+) years of project management experience
  • Experience completing or leading quality improvement initiatives in a clinical setting
  • Knowledge of the Patient Centered Medical Home (PCMH) and Quality Payment Program (QPP) initiatives
  • Outpatient primary care experience and familiarity with hypertension, diabetes, smoking and other cardiovascular-related priority health areas
  • Ability to work collaboratively with physicians, staff, and external organizations
  • Strong computer skills
  • Electronic health record and training experience
  • Excellent time management and communication skills

 

There is a potential for this position to transition to DOHMH and therefore candidates must meet DOHMH eligibility requirements, including NYC residency.
