Medical - Registered Nurse

Summary:
The Registered Nurse (RN) provides professional nursing care to patients at a San Diego American Indian Health Center. The RN collaborates with the interdisciplinary team, including physicians, nurse practitioners, medical assistants, and support staff, to deliver high-quality, patient-centered care. The RN supports clinic operations and ensures compliance with regulatory standards while maintaining a focus on preventive care and chronic disease management.
Essential Duties and Responsibilities:
Patient Care
- Performs appropriate treatments as ordered by physician in an accurate and timely manner.
- Assess, plan, and implement patient care in accordance with SDAIHC standards, policies, and procedures.
- Conduct triage, manage chronic conditions, and administer medications or treatments as prescribed.
- Provide education and counseling to patients on various health conditions and preventive care measures.
- Assist in minor procedures and perform wound care, immunizations, and diagnostic testing (e.g., blood pressure, glucose levels).
- Support the care of patients with chronic illnesses (e.g., diabetes, hypertension) through case management.
- Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family.
- Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
- May assists clinic manager with disciplinary actions, and performance evaluations.
- Checks and oversees maintenance of clinic logs including refrigerator and freezer temperatures twice daily (AM/PM), room cleaning, hazardous waste, and expiration dates.
- Assists/oversees medical supplies inventory and materials.
- Ensures company policy is being adhered to by MA's, take corrective action when necessary.
- Assists clinic manager with HEDIS, UDS, GPRA measures.
- Assists clinic manager in facilitating inter-department communication.
- Performs other duties as assigned.
Care Coordination
- Collaborate with providers and clinical staff to coordinate patient care, including follow-up appointments and referrals.
- Participate in care team meetings to ensure quality and continuity of care.
- Provide patient education regarding medications, treatments, and health maintenance.
Health Screenings and Preventive Services
- Participate in community outreach efforts to promote health education and preventive care.
- Administer vaccines and screenings per federal, state, and organizational guidelines.
Documentation and Compliance
- Maintain accurate and timely documentation in the Electronic Health Record (EHR) system.
- Ensure compliance with FQHC standards, HIPAA, OSHA, and infection control policies.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
- Demonstrates proficiency in the use of the clinic’s E.H.R, including appropriate documentation of visits, telephone notes, medical Rx refill requests, referral process, and data entry into patient’s chart.
- Checks CAIR for up-to-date IZ records.
Patient Advocacy
- Act as an advocate for patients, ensuring their rights are respected and their health needs are met.
- Provide culturally sensitive care that recognizes the unique needs of underserved and vulnerable populations.
Qualifications
Education:
- Associate or bachelor’s degree in nursing from an accredited nursing program.
- Bachelor’s in nursing preferred.
Licensure/Certification:
- Current CA Registered Nurse (RN) license in good standing.
- CPR BLS Provider (AHA).
Experience:
- Minimum of 1-2 years of nursing experience in an outpatient, primary care, or community health setting, preferably at an FQHC or similar clinic.
- Experience with EHR systems (e.g., ECw is a plus.
Work Environment
This job operates in a clinical setting, and the RN will be exposed to patients, bodily fluids, and communicable diseases. The RN may need to lift and support patients and work in a fast-paced environment. The role may also involve participating in community outreach programs or events.

- Immediately reports unsafe working conditions.

- Maintains privacy and security of all patients, employee, and volunteer information and access to such information. Such information is accessed on a need-to-know basis for business purposes only.

Flexibility: Available for all shifts and, when required, able to work evenings and weekends
