Job Opportunity: Part-Time Community Health Worker
Location: YMCA of Greater Indianapolis
Position Type: Part-Time
About the Role:
Are you passionate about promoting health and wellness in your community? The YMCA of Greater Indianapolis is looking for a dedicated and enthusiastic Community Health Worker (CHW) to join our Healthy Living Team on a part-time basis. In this role, you'll support individuals and families by delivering vital health education and assistance, focusing particularly on our Diabetes Prevention Program and other chronic disease prevention initiatives.
Key Responsibilities:
- Program Delivery: Lead and support the Diabetes Prevention Program and assist with other chronic disease prevention programs.
- Participant Support: Provide guidance and motivation to participants, helping them achieve their health and wellness goals.
- Community Outreach: Assist in recruiting and enrolling participants, addressing individual and community needs, and providing resource referrals.
- Data Management: Accurately enter and manage participant data, ensuring privacy and compliance with all regulations.
- Educational Instruction: Teach classes on health and wellness topics, making sure the environment is engaging and non-competitive.
Requirements:
- Education: High school diploma or equivalent.
- Certification: Existing Community Health Worker certification preferred, or willingness to complete YMCA-provided training and certification within the first 6 months of employment.
- Skills: Strong communication and organizational skills, experience in leading classes, and a basic understanding of health and wellness principles.
- Experience: At least 1 year of experience working with special populations is preferred.
Schedule:
- Hours: This is a part-time position. You will need to be available to lead programs during evenings and at various YMCA locations. Flexibility is important.
Additional Information:
- Training: YMCA-provided training in HIPAA, CPR AED, First Aid, and other relevant areas must be completed within the first 60 days of employment.

- Participation: Engage in ongoing education and attend staff meetings and trainings related to the Chronic Disease Programs.
