The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

 

PROGRAM OVERVIEW

DOHMH Queens Neighborhood Health is leading a community health promotion program to reduce racial/ethnic disparities and premature mortality by growing public health capacity in partnership with community-based organizations (CBOS) in the most disinvested neighborhoods in Queens. The Program will use a multi-pronged community engagement strategy to increase access to disease prevention, treatment, and relevant services; address health risk factors including chronic disease, social determinants of health and structural racism; and build collective action to address racial disparities and resource needs throughout long-term recovery. 

 

POSITION OVERVIEW

The Community Health Worker will support community-based organizations (CBOs) with deep neighborhood knowledge to develop community capacity in neighborhoods most impacted by COVID-19 and long-standing racial/ethnic inequities in Queens (Jamaica, Corona and Far Rockaway) by increasing access to health services, improving overall health outcomes, and addressing social needs of the neighborhood. This program will expand DOHMH long-term model to provide a sustainable, organized backbone of CBO partnerships that can address health disparities at the neighborhood level, coordinate long-term community recovery, and increase community preparedness for future crises. The Community Health Worker will report to the Queens Neighborhood Health Program Manager and be responsible for facilitating community engagement activities in Corona/Elmhurst, Jamaica, and Far Rockaways. This position will feature a primary focus on the catchment area of Jamaica, with a strong preference for those who have experience with community work in the area or are from the community themselves.

 

RESPONSIBILITIES

  • CHWs will lead community engagement activities, such as health education tabling at health fairs and other community events.
  • CHWs will prepare educational material, personal protective equipment, and program incentives to be used. 

•     CHWs will support program promotion and activities and keep up to date on health education topics (i.e. latest guidelines for COVID-19 and monkeypox).

•     CHWs will collect data from canvassing and community engagement work

  • CHWs will provide health education to community residents.
  • CHWs will develop and strengthen community partner relationships.

•     Must be professional and consistent with strong follow up skills

  • Must be willing to travel for community engagement activities as well as work on weekends and evenings depending on community events.
  • Support other program activities as needed and attend professional trainings.

 

QUALIFICATIONS

  • Bachelor’s degree preferred or minimum four years of experience plus some college.
  • Previous experience in community engagement activities preferably in Corona/Elmhurst, Jamaica, and Far Rockaways
  • Bilingual with strong preference for Haitian Creole
  • Strong relationship builder with excellent interpersonal and communication skills
  • Strong and proven outreach skills
  • Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred.
  • Demonstrated outreach innovations.
  • Experience working with marginalized populations preferred.

SALARY AND BENEFITS

FPHNYC offers a comprehensive benefit package.

  • Salary range is $77,000 to $85,000
