Community Health Worker
PROGRAM OVERVIEW
The NYC Department of Health and Mental Hygiene (NYC Health Department) is seeking to hire a Community Health Worker to coordinate tobacco-related efforts at the Tremont and East Harlem Neighborhood Health Action Centers. This role sits in the Tobacco Policy and Programs Unit in the Bureau of Chronic Disease Prevention in the Center for Health Equity & Community Wellness (CHECW). The candidate will also work closely with the Bureaus of Bronx and Harlem Neighborhoods Health in CHECW. CHECW seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW’s work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is a division of the NYC Health Department.
POSITION OVERVIEW
The Community Health Worker will provide tobacco treatment services and coordinate related outreach and evaluation activities at the Bronx and Harlem Neighborhood Health Action Centers, as well as support tobacco outreach activities out the Bureau of Chronic Disease Prevention. This role reports to the Tobacco Treatment Outreach Manager within the Tobacco Policy and Programs Unit in the Bureau of Chronic Disease Prevention and will coordinate closely with the Director(s) of Community Engagement at the Bureaus of Bronx and Harlem Neighborhoods Health.
RESPONSIBILITIES
- Assist with planning and executing tobacco treatment and outreach activities in diverse settings, including ongoing community events (some evening and weekend work may be required)
- Screen clients for tobacco use, distribute nicotine replacement therapy and other materials, and provide coaching to clients who smoke or vape
- Conduct follow-ups with clients who received tobacco treatment medications or resources
- Refer clients to tobacco treatment programs or healthcare providers for longer-term treatment
- Conduct outreach, develop relationships, and engage and strengthen new and existing collaboration with community residents, organizations, and key stakeholder program partners in the South Bronx and East Harlem by sharing information and resources to address tobacco use in the community
- Track and manage community outreach, treatment, and programmatic efforts
- Support and participate in other related evaluation efforts
- Support other tobacco-related activities and action center events, as needed
QUALIFICATIONS
- High School graduate or some college (or Associates)
- At least 6 months experience in human services or a related field
- Experience with community engagement/outreach
- Experience with data entry and tracking
- Strong interpersonal skills and the ability to build trust and maintain relationships with residents and community partners
- Quick learner and self-starter
- Works well independently and within team
- Strong organizational skills with good follow-through to complete assignments within specified timeframes
- Ability to travel through the five boroughs for outreach events and manage up to 10 pounds of materials (medications, publications) for distribution
Preferences:
- Preferred client experience, providing navigation or referral support for healthcare or social services.
- Bilingual in Spanish or Mandarin
- Experience working, living, and/or engaging in the South Bronx or East Harlem
- Knowledge of or interest in tobacco-related issues
- Ability to understand the concepts of social determinants of health, institutional racism, structural racism, health inequities and their impact on underserved and underrepresented communities
- Demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g., people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)
- Experience with Microsoft Office Suite, including Excel, Word, Forms, PowerPoint, and Outlook
LOCATION
Time will be split between different Health Department Offices (Tremont Action Center [1826 Arthur Avenue, Bronx, NY 10457], the East Harlem Action Center [158 E. 115th St, New York, NY 10029], Gotham Center [42-09 28th Street, Long Island City, NY, 11101], as well as events in the community.
TIME COMMITMENT
- 35 hours per week
- November 2024 through June 2025
- Main schedule will be M-F 9:00 am to 5:00 pm, but should be willing to switch hours to work some evenings and weekends to attend community events (2-4 times monthly)
SALARY AND BENEFITS
Pay rate is $35/hour
