Chronic Disease Community Engagement and Planning Advisor

The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW

Since January 2020, DOHMH has been fighting the COVID-19 pandemic- activities include but are not limited to surveillance & epidemiological investigations; lab testing; engaging and providing guidance to healthcare partners; ensuring the safety & health of responders; providing timely and accurate information to the public; promoting the emotional health of New Yorkers and coordinating citywide COVID-19 vaccine administration. DOHMH works closely with community-based organizations (CBOs) to increase access to and awareness of COVID-19 prevention, treatment, and vaccination services. Consistent with the Health Department’s decades long commitment to place-based investments, new work is focused on neighborhoods who have been hardest hit by the COVID-19 pandemic, have low vaccination rates, and have been harmed by racial and ethnic health inequities. DOHMH seeks to continue place-based anti-racism interventions toward COVID-19 recovery and chronic disease prevention over the coming two years through continued investment in the Public Health Corps. 

POSITION OVERVIEW

The DOHMH is seeking a Chronic Disease Community Engagement and Planning Advisor, to align multiple streams of environmental asthma trigger mitigation strategies and chronic disease prevention work until June 2026.

This role sits in the Bureau of Bronx Neighborhood Health in the Center for Health Equity & Community Wellness (CHECW), which aims to eliminate racial inequities resulting in premature mortality, with a focus on chronic disease, by addressing the social and environmental factors that impact health. CHECW unifies and strengthens the Department's lines of work directed at eliminating racial inequities for preventable health conditions, which are rooted in historical and contemporary injustices and discrimination, including racism. The Bureau of Bronx Neighborhood Health is seeking to hire a Chronic Disease Community Engagement and Planning Advisor who will be responsible for promoting and coordinating environmental asthma trigger mitigation strategies   to improve the health status of neighborhoods disproportionately burdened with asthma. The Chronic Disease Community Engagement and Planning Advisor, with latitude for independent initiative and judgment, performs responsible work in any phase of community programs and engagement. This includes providing community engagement and programmatic support in promoting health equity and improved asthma management throughout the Bronx. The Chronic Disease Community Engagement and Planning Advisor will align activities, timelines, and deliverables across multiple workstreams in the borough and support this work.

In the event of a public health emergency as deemed by the agency (public health outbreak, natural disaster, etc.) employees may be mandated to assume an emergency response role. In these rare instances and when notified, staff will be re-assigned from their regular day-to-day duties as noted in this job description to take on another role considered necessary by the agency. When this occurs, all staff are required to comply with the change in assignment and must be prepared to be called upon promptly.

RESPONSIBILITIES

• Oversee the Bureau of the Bronx Neighborhood Health’s Integrated Pest Management (IPM) program by: establishing, and maintaining appropriate systems for measuring necessary aspects of IPM for reporting; preparing documents related to scripts, budgeting and finance; facilitating an effective relationship between the DOHMH and the selected IPM vendor(s) to ensure high quality service delivery on behalf of our clients, including regular unscheduled IPM home visits to verify the program’s quality standards; developing and maintaining data collection systems including databases, surveys, inspector forms, monthly referral reports, and evaluation processes; promoting the Bronx IPM program; facilitating IPM requests and successful completion of the Bronx IPM referral pathway; streamlining IPM referrals across other DOHMH IPM programs (Healthy Homes & Medicaid Together); supporting Medicaid Together’ s engagement and referrals in the Bronx by streamlining referrals/allergy testing and fostering strategic partnerships with clinical settings; and developing and implementing an evaluation plan, in collaboration with our Bureau’s Research and Evaluation team, to assess the IPM program’s impact.
• Supervise and support CHW’s responsible for IPM program implementation in professional development, community outreach, engagement, and facilitating referrals.
• Supervise CHW responsible for Nicotine Replacement Therapy program in the Bureau of Bronx Neighborhood Health Catchment area. Coordinate with Nicotine Replacement Therapy program leads within the bureau and across the agency.
• Remain up to date on all things pediatric asthma.
• Plan and facilitate health awareness and promotion activities within the community, including, community outreach, workshops, programming, and presentations in diverse settings around asthma management, chronic disease prevention, health equity, and programmatic services (some evening and weekends work required).Engage community partners and local organizations, including small businesses, civic organizations, community groups, not-for-profits, and faith-based organizations and provide them with data and other resources for improving their community's health and/or asthma management. Develop strong relationships and strengthen collaboration with community organizations and program partners including the NYC government agencies, community based and private organizations, medical and health providers working on improving health equity in the Bronx. Strategize, develop, and implement a community engagement strategy focused on leveraging community-based organizations/stakeholders to address asthma management and trigger prevention. Collaborate with bureau leadership to align environmental asthma trigger reduction efforts with bureau’s goals and objectives. Track, maintain and monitor community engagement and programmatic efforts and enter all relevant qualitative and quantitative data into database platforms. Assist with program administration for community engagement work and provide clerical support as needed.

• Support other programmatic needs within the Bureau including research and evaluation, food security, programmatic referrals and navigation, coalition support, and capacity building.

QUALIFICATIONS

• A Baccalaureate degree from an accredited college and 2 years of full-time satisfactory experience in community work
• A high school diploma/GED and 3-4 years of full-time satisfactory experience in community work
• Quick learner and able to work in a fast-paced environment and take initiative

• Works well independently and within team
• Strong community engagement experience
• Detail oriented, familiarity tracking and monitoring programmatic data
• Excellent written and oral presentation skills
• Experience working and/or engaging in priority neighborhoods
• Strong interpersonal skills and the ability to build trust and maintain relationships with community partners
• Ability to effectively communicate in public forums and with diverse audiences
• Strong organizational skills with good follow-through to complete assignments within specified timeframes and juggle several tasks simultaneously
• Proficiency in Microsoft software applications with strong emphasis on Outlook, PowerPoint, Word, and Excel

PREFERENCES

• Experience with Salesforce and Tableau a plus
• Bilingual in Spanish or French
• Demonstrable experience in addressing drivers of COVID-19 vaccine hesitancy and chronic disease
• Experience in emergency recovery and resilience work
• In-depth knowledge of the borough community stakeholders 
• Ability to understand the concepts of social determinants of health, institutional racism, structural racism, health inequities and their impact on underserved and underrepresented communities
• Demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g., people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)

35 hours per week.

This position will be funded through June 30, 2026, with possibility of extension.
