Nutritionist/Dietician Facilitator for Diabetes Self-Management Education and Support (DSMES) Curriculum at Urban Health Plan in Collaboration with NYC Dept of Health and Mental Hygiene

PROGRAM OVERVIEW
The New York City Department of Health and Mental Hygiene’s Bureau of Bronx Neighborhood Health is looking to identify a nutritionist/dietician willing to deliver a 5-part workshop series for people at risk for, interested in, or diagnosed with Type II diabetes using Diabetes Self-Management Education and Support curriculum. This curriculum will be delivered in conjunction with Mas Verduras, a prescription produce program funded by the United States Department of Agriculture. Mas Verduras is a partnership between Urban Health Plan (UHP) and DOHMH that will provide financial benefit for fresh fruits and vegetables to UHP patients who are food insecure, enrolled in Medicaid, and have uncontrolled Type II diabetes. The DSMES courses will first be offered to Mas Verduras participants but may include other Urban Health Plan patients if space allows.
POSITION OVERVIEW
Over the two-year period, four cohorts of workshops will be offered virtually and four will be offered in-person at Urban Health Plan in the Bronx, NY. Workshops may be offered in evening or weekend hours. The cohorts would start in October 2024 at the earliest.
RESPONSIBILITIES
The nutritionist/dietician will be required to:
- Take part in 3-hour training for DSMES Facilitators.
- Deliver each part of the 5-session workshop (each session is 90-120 minutes).
- Workshops will be held either in-person at Urban Health Plan or virtually (held via Microsoft Teams) depending on program need.
- Take part in class preparation reviewing pre-completed participant assessment surveys, including self-assessments at the start of the course and satisfaction surveys at the end of the course.
- Nutritionist/dietician will comply with all relevant procedures that DOHMH may provide in relation to this service, including HIPAA protocols.
QUALIFICATIONS
- A licensed Registered Dietitian/CDN
OR
- Nutrition master’s level graduate without a license
SALARY RANGE
Nutritionist/Dietician will be paid $1,000 for each cohort (5 session workshop series) up to a max of $8,000.
