Formosa Plastics is a privately held company headquartered in Livingston, New Jersey. Our core business, producing plastic resins and petrochemicals, takes place at three wholly-owned chemical manufacturing subsidiaries located in Delaware City, Delaware, Baton Rouge, Louisiana and Point Comfort, Texas.
I. POSITION OBJECTIVE
Conduct root cause analysis investigations. Ensure accuracy and completeness of investigations. Serve as an Investigation Team Leader. Be able to utilize a database such as KMS to record investigation details. Assist in verifying incident types are correctly identified and finalize investigation reports. Demonstrate analytical and problem-solving abilities. Conduct investigation interviews to obtain evidence needed for the investigation.
II. MAJOR AREAS OF ACCOUNTABILITY
Lead and Facilitate Investigations: Work as a problem solver and facilitate incidents as requested at the site level. Present finalized investigations to management, as requested.
Quality Assurance: Conduct QA on investigations submitted by site team leaders to ensure consistency in report formatting and usage of methodology.
KMS Reviewer: Serve as one of the reviewers of TX incidents initiated/closed in KMS.
Support: Assist site-level team leaders, as needed, to improve competency and ensure completeness of detailed investigations. Provide day-to-day support to management for incident investigation. Support incident investigation training activities. Assist the KMS team with incident trending, as needed. Work with 3rd party consultants, as needed to support incident investigation goals. Assist with incident investigation and reporting procedure revisions including application of tools and resources. Assist with TX site quarterly incident investigation networking meetings.
Train: Conduct one-on-one training sessions with site facilitators to aid in educating and expanding their knowledge which ultimately improves site investigation quality.
THM: Promote cleanliness and order through the implementation of the Total Housekeeping Maintenance program. Emphasize Nothing Touch Ground program and No Visual Pollution program. Maintain the plant in like new condition.
III. QUALIFICATIONS
- A minimum of six (6) to ten (10) years of applicable process chemistry or incident investigation experience within a petrochemical or refining site depending on the level of education. Knowledge of EHS and PSM procedures. Preferred experience with leading incident investigations utilizing Cause Mapping or root cause analysis methodology.
- High School diploma or equivalent required with (10) years of above-mentioned experience. Bachelor's degree [preferred] in engineering disciplines, safety, or PSM-related disciplines with 6 years of experience or an Associate’s Degree with 8 years of above-mentioned experience.
