Shift: Full-Time, Monday-Friday 3pm-11pm

What You’ll Do:

• Support consumers in achieving personal care goals and addressing ongoing challenges
• Ensure adherence to Adult Mental Health Rehabilitation Services (AMHRS) standards
• Respond promptly to consumer concerns, ensuring high satisfaction
• Educate consumers on medications, track logs, and conduct monthly audits
• Coordinate services based on individual needs
• Deliver high-quality care in line with Individual Rehabilitation Plans (IRP)
• Accurately document services within required timeframes using electronic health records

Job Requirements:

• Bachelor’s Degree in Mental Health or Human Services, OR
• A combination of 1+ years of college and related life or work experience totaling 4 years, OR
• High School/GED plus 4+ years of related work or life experience
• Intermediate computer skills with the ability to learn new programs
• Valid Driver’s License and an acceptable driving record
• Strong verbal and written communication skills
• Compassionate, flexible, and client-focused approach
