What You’ll Do:

• Provide case management for HUD programs, ensuring compliance with DMHAS CSS guidelines.
• Spend 50% of your time offering face-to-face supportive services in the field.
• Coordinate care with clinical teams and external agencies.
• Help consumers sustain wellness and recovery beyond program enrollment.
• Contribute to service improvements and consumer education.
• Ensure compliance with licensing and regulatory standards.

Job Requirements:

• Bachelor’s Degree in Social Work, Psychology, or related field.
• Experience with community-based services preferred.
• Strong computer skills (Word, Excel).
• Valid Driver’s License and acceptable driving record.
• Excellent communication, planning, and organizational skills.
