What You’ll Do:

  • Provide ongoing support to individuals in their community to help them function independently and access mental health services.
  • Engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
  • Assist consumers in accessing needed services and ensure transportation to appointments.
  • Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
  • Assess the need for crisis intervention and assist in psychiatric emergency situations.
  • Encourage wellness and recovery while maintaining individual dignity and respect.
  • Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.

Job Requirements:

  • Bachelor’s Degree in Social Work, Psychology, or a related field from an accredited college or university.
  • Experience with community-based mental health services is preferred.
  • Working knowledge of computer software, including word processing and Excel.
  • Valid Driver’s License and an acceptable driving record.
  • Strong verbal and written communication skills.
  • Ability to work effectively with individuals and teams.
  • Compassionate, client-centered approach to care.
